Our Care Navigators are a point of contact to provide guidance and information; ensuring that appointments are arranged in your best interest so you are receiving the right care at the right time.

Our Care Coordinator helps to provide continuity and improved communication when multiple services are involved in your care or you have a long-term health need. They can help to manage any existing referrals and streamline your care. They’ll also direct you to the most appropriate contact in our GP at Hand network, giving you easy access to the assessments and advice you need.

If you’re a GP at Hand patient you can send our Support Team a message or discuss with your clinician on your next appointment. If appropriate, they’ll refer you and then a member of the team will be in touch to arrange an appointment.
During your first appointment, you’ll have a one-to-one session with a member of the team. The appointment will be over video or phone. Together, you’ll explore the issues surrounding your care, agree objectives to work towards and arrange any follow-ups.
We have appointments available Monday-Friday 9:00am - 18:00pm. Our team members are non-clinical and do not replace clinical advice or interventions. You can still book with any of our clinicians through the eMed app or web portal as part of your normal GP at Hand service.
GP at Hand patients have access to choose a clinician when booking an appointment in the app. You can find out more about each clinician by viewing their bio in the booking screens. You can also book with clinicians you’ve seen before.
