You can update your personal details in the eMed App or eMed web portal if you’ve changed your:
If you’ve changed your name, you’ll need to contact our Support Team so they can update your medical record.
Changing your address
To change your address, you’ll need to provide your new:
- house number or name
- street name
- town
- postcode
We’ll update your NHS medical record with your new address. This usually takes 5 working days.
If you no longer live or work within 40 minutes travel time of one of our clinics, you should re-register with a local GP. This is so you can easily and safely access face to face care if it is required.
If you’re using the eMed App
- Go to Me (
) - Tap Account details, then Edit
- Enter your new details and tap Save
If you’re using the eMed web portal
- Log in in to your account
- Select your account icon (top right with your initials)
- Go to Account
- Select Personal Details, then Edit
- Enter your new details and click Save
Changing your phone number or email

- Enter your new details and tap Save
If you’re using the eMed web portal

- Go to Account
- Select Personal Details, then Edit

Changing your name
To change your name on your eMed account and NHS medical record, fill in our contact form. You'll need to include:
- Your previous name, along with your date of birth and full address (so we can locate and access your medical record)
- Your current registered name
- A copy of your deed poll or marriage certificate with your current registered name (as an attachment)
